TERMS AND CONDITIONS

REGISTRATION BY MAIL

Registrations may be processed on-line by credit card using either Visa or MasterCard at www.PublicWorksforPublicLearning.info, or the registration form may be downloaded as a PDF, completed, and mailed in with a check to:

We cannot accept purchase orders or vouchers as payment. Registrations will be closed once we've reached capacity.

Every registration will be sent a confirmation by email. If not received, please contact the CUREE office at 510-665-3529.

REFUNDS

If you are unable to attend, an alternate may be designated to use your registration if you contact the CUREE office with the alternate's name and contact information. Refunds requested after May 1, 2012, but before June 1, 2012, will incur a $50 cancellation fee. There are no refunds after June 1, 2012.

LIABILITY WAIVER AND RELEASE

In consideration of being allowed to participate in any way in the Golden Gate Bridge International Conference on Public Works for Public Learning any related events and activities, I, the undersigned, acknowledge, appreciate, and agree that I am voluntarily undertaking participation in this conference; by doing so I assume all risk and take full responsibility for my own well-being. I am fully aware that possible property damage, physical injury, illness or death may occur as a result of my participation in these events and activities. I forever release the Golden Gate Bridge Highway Transportation District (GGBHTD) and the Consortium of Universities for Research on Earthquake Engineering (CUREE), its directors, officers, employees, volunteers, agents, contractors, and representatives (collectively "Releasees") from any and all actions, claims, or demands that I, my family or heirs now have or may have in the future related to my participation in these activities.

PHOTOGRAPHS

I agree and acknowledge that GGBHTD and CUREE plan to take photographs at the Golden Gate Bridge International Conference on Public Works for Public Learning and reproduce them in educational, news or promotional material, whether in print, electronic or other media, including the GGBHTD, CUREE and/or National Science Foundation (NSF), herein called the conference representatives, website. By participating in the conference you grant GGBHTD, CUREE and NSF the right to use your name, photograph and biography for such purposes.

I am aware this is a release of liability and rights of use related to photographs; a contract between the conference representatives and myself. I have read, understand and agree to these terms and am entering into this agreement.
